叶为民,男,汉族,1968年12月出生,福建尤溪人,1991年8月参加工作。瑞典卡罗琳斯卡医学院流行病学专业毕业,研究生学历、教授、博士生导师,欧洲科学院院士,国家级人才计划专家,瑞典卡罗琳斯卡医学院终身教授。现任福建医科大学副校长并兼任公共卫生学院院长、群体医学研究院院长。长期从事肿瘤病因流行病学与防控研究,特别是对鼻咽癌,食管癌,胃癌和胰腺癌开展了系统研究。在国际期刊上共发表研究论文530余篇,他引40000余次,H因子106(Google Scholar)。2005年获挪威奥斯陆大学Anders Jahre青年医学研究奖。2016年获欧盟研究理事会杰出人才基金。近五年主持国家重点研发计划项目、国家自然科学基金重点项目、区域创新发展联合基金项目、福建省科技厅中央引导地方科技发展专项资金项目等国家级、省部级项目10余项。担任《Phenomics》等3个国际杂志的编辑,50多个杂志及基金的评审专家。

长期从事肿瘤病因流行病学与防控研究,特别是对鼻咽癌,食管癌,胃癌和胰腺癌开展了系统研究。

福建医科大学副校长、公共卫生学院院长,瑞典卡罗琳斯卡医学院终身教授,国家级人才计划专家,中华预防医学会肿瘤预防与控制专委会副主任委员,第三批全国高校黄大年式教师团队(公共卫生与预防医学)负责人。
长期从事慢性病的病因流行病学与防控研究工作,在慢性病病因学研究上拥有多项成果与丰富经验。相关研究在BMJ、JAMA Oncology、Gastroenterology、Gut等期刊发表480余篇,他引36000余次,H因子100(Google Scholar)。近五年主持国家自然科学基金重点项目、区域创新发展联合基金项目等国家级、省部级项目10余项。

倾力拓荒学科空白,重塑癌症防治格局
追风赶月莫停留,平芜尽处是青山。今年,是叶为民步入医学殿堂的第41年。而回溯到最初,41年前的他未曾料想,一个冥冥之中的选择让他与流行病学就此结缘一生。
5年本科、3年硕士、4年博士……叶为民自1983年考入福建医科大学卫生系就读开始,就一直奋力奔跑在艰辛而漫长的医学路上,将全部精力都投入课题研究、学业钻研之中,最终获得瑞典卡罗琳斯卡医学院博士学位。凭借丰富的现场经验、广博的理论知识和刻苦钻研、勇于创新的精神,叶为民先后担任日本国立癌症研究中心访问学者,瑞典卡罗琳斯卡医学院研究员、教授,福建医科大学副校长、公共卫生学院院长等职务。
心怀国之大者,情系民之所盼。“我们团队的座右铭是顶天立地为人民。鉴于癌症在我国防控形势的严峻,要集中优势力量开展疑难高发癌症防治专项重点攻关,推动建立防、监、治、控链式周期管理体系。”叶为民领导的流行病学团队极大地推动了癌症发生发展机制研究和突破性关键技术的研发进程。
从谈癌色变到谈癌思变,叶为民一直在努力。为进一步提高人们对肿瘤防治的了解和认识,创新和完善癌症防治工作,叶为民团队深入分析三次全国死因回顾性调查的公开数据和2022年国家癌症登记中心公布的最新(2016年)癌症登记数据,形象绘制出我国首份主要癌症死亡原因及死亡率的时空变化图。研究发现,从1973年到2016年我国胃癌、食管癌、宫颈癌的死亡率出现了明显下降,而肺癌、结直肠癌和女性乳腺癌的死亡率逐渐上升;农村地区与城市地区的癌症死亡模式存在时间差;近30年,尽管首次恶性肿瘤死因调查显示前三位的癌症(胃癌、食管癌和肝癌)的大多数高发地区(点)对应死亡率有所下降,但各地区的各个癌种的死亡率仍高于同期全国的对应癌症总死亡率水平……相关研究成果已发表在Cancer Communications杂志上。
在我国,每年新发胃癌病例和死亡病例数都接近全球的一半,而幽门螺杆菌感染被认为是胃癌发病过程中最重要的风险因素。从胃癌高发区福州长乐的乡间地头到福建医科大学的实验室,叶为民团队以一项长达26.5年的随机对照试验打响了胃癌“阻击战”,科学评估了根除幽门螺杆菌对胃癌发病率和死亡率的长期影响。这项始于1994年7月并随访至2020年12月的前瞻性、随机、安慰剂对照试验表明,幽门螺杆菌根除治疗可显著降低胃癌长期发病风险,这种获益在没有癌前病变或消化不良的个体中表现则更为明显,风险降低程度可分别达到63%和56%。相关研究成果于2022年3月在Gastroenterology在线发表。
为构筑肿瘤“防治网”,当好人民健康“守门人”,身为消化道肿瘤防治领域的顶尖学者,叶为民还利用多组学数据,评估胃微生物组、炎症蛋白组和DNA甲基化改变之间的关系及其在胃癌前病变中的作用,揭示了炎症蛋白在胃微生物群失调与DNA甲基化改变的关联间具有中介作用,为胃微生物群失调促进胃癌前病变发生的机制提供了新的见解,并在Phenomics杂志上发表了题为“Inflammation as a Mediator of Microbiome Dysbiosis‑Associated DNA Methylation Changes in Gastric Premalignant Lesions”的论文。
“要先到病患身边了解他们需要什么,再回到实验室研究怎么解决。随着基础研究的进展,我们对癌症的了解越来越多,这为预防与早发现癌症提供了更多选择和可能。”面对人民生命健康,叶为民始终以与时俱进的精神、革故鼎新的勇气、坚韧不拔的定力,不断向科学技术广度和深度进军,引领性科研成果加速涌现,为我国医学发展作出了巨大贡献。

造福一方百姓,诠释医者仁心
心怀苍生,大爱无疆;坚韧不拔,矢志不渝;肩鸿任钜,砥砺前行……身处海外二十余年,叶为民坚持不改国籍,始终心系祖国与母校,带领团队聚焦国际学科前沿,紧密围绕健康中国战略,长期致力于公共卫生领域的基础理论和应用研究,坚持“四个面向”搭建科研服务平台,在无畏付出、无私奉献中为爱国奋斗写下了生动注脚。
癌症是多种因素长期综合作用的结果,其发生与环境因素及生活方式密切相关。福建省地处东南沿海消化道恶性肿瘤高发带中,学有所成的叶为民主动回到家乡谋求破局之策,以老百姓的就医需求为出发点和落脚点,依托高水平医学院校和省级高水平医院的资源及平台,推动建立“府-院-校”共建新模式,积极探索该地区肿瘤等慢性病发病机制,开发、应用和推广防控策略和工具,在福清、莆田、泉港等地建成7.1万人的健康人群队列和1万人的临床专病队列,形成以福建省健康人群队列和数字福建肿瘤大数据研究所为依托的医疗大数据整理、分析实训平台。
“近年来,在基因组学和测序技术的飞速发展的背景下,大型自然人群队列在精准医疗领域的重要价值开始逐步凸显,日益成为国际生命科学与疾病领域的前沿与焦点。”自然人群队列研究是指通过对普通人群进行体格检查、生物样本采集和问卷调查,并进行长期持续的动态追踪观察,掌握人口健康状况、阐述疾病的发展史、揭示疾病的病因、引导实验设计、评价预防效果,将知识转化为临床和人群早期筛查诊断和干预策略,从而提高疾病的防治水平,降低社会卫生负担,为下一步针对性制定卫生方针政策提供科学依据,是疾病防控领域的重大基础战略工程。
叶为民介绍称,以大型队列为基础,整合生物样本库、遗传和基因组数据库、表型数据库等建立越来越大的生物银行(Biobank)正日渐成为全球的趋势。表型组与大型队列存在取与舍的问题,而精细化人群队列在有限的可接受的人群队列中,采集尽量多的表型组,以实现数量和质量的均衡。福建省自然人群队列项目规划覆盖20万人群,致力于实现精细化、高应答和低失访,“随访是队列研究成败的关键,大数据技术的发展为疾病随访率的提升提供了良好的契机,福建队列也在数据中心建设方面积累了一些经验。”
“想要做好现场,不能只在办公室待着学理论,还要去基层中实践,真正去掌握流调技能、了解数据收集过程,解决实际中遇到的困难。”回忆起队列建设时,叶为民团队的教师表示这是叶为民反复强调多次的。
作为福建省沿海地区,福清市是糖尿病、高血压、甲状腺肿瘤等疾病相对多发地区之一,亟需探究当地危险因素并采取针对性措施。2021年11月,由福建医科大学与福清市政府组建的福建队列研究中心福清基地揭牌成立,联合开展大型前瞻性自然人群队列研究,科学探索肿瘤及多种慢性病的发生和发展规律,开展慢性病的病因预防和临床诊疗研究,为慢性病的早期筛查、诊断、个体化治疗和药物研发提供科学依据。目前,已对福清市高山镇、三山镇、东瀚镇、沙埔镇、港头镇等地的3.9万余名常住居民进行健康评估,并有针对性地开展疾病的诊疗指导,让“三福之地”群众获得感、幸福感、安全感更加充实、更有保障、更可持续。经过一系列的探索实践,叶为民推动形成了重大慢性病防控的“福清模式”,为福建省医疗卫生事业发展奠定坚实的基础,这也意味着,更高质量、更高水平的医疗服务将极大地惠及当地百姓。2023年12月,叶为民获评“福清市第二届荣誉市民”称号。
正因见证过太多“发现一例早癌、挽救一条生命、幸福一个家庭”的案例,叶为民早就将基层肿瘤防治当作了自己一生的事业。2020年12月2日,福建省队列研究泉州基地挂牌成立,由此补齐泉州地区消化道肿瘤科研、预防、综合治疗的短板,推动泉州生命科学领域研究和医疗健康核心技术取得突破,进一步带动提升泉港综合办医水平,为泉港医疗卫生事业高质量发展注入新的动能。
尤为值得一提的是,叶为民牵头推动建立以医院来源的患者医疗数据为核心的大数据平台,汇聚福州市居民健康医疗相关数据,打通队列数据与医疗数据接口,为后续诊疗大数据的应用开发和人群前瞻性队列建设奠定基础。
“有目标、有韧劲,能够用发展的眼光看待一切事物和人。”这是业界与师生对叶为民的一致评价。作为叶为民的第一个博士毕业生,鄢灵君回忆称,“在进行胃癌课题研究时,我们对长乐现场的研究对象进行长期随访。叶教授要求我们一家家医院地去核实患者的医疗记录,且要求数据分析全程确保对数据进行更严格的清洗和校验。这次经历教会了我,数据的质量和准确性是科学研究的基石,我们必须始终保持批判性思维和严谨的态度。”
心中有民,行之为民。叶为民希望,持续推进和优化疾病预警机制,融合队列资源,关注婴幼儿、成年人、老年人全生命周期的健康监测,探索创新更加精准的疾病防控举措。

立德树人传薪火,培根铸魂育英才
一直以来,叶为民坚守为党育人、为国育才的初心使命,以“四有好老师”“四个引路人”“四个相统一”为目标,融思想教育、专业教育、职业精神培养为一体,激励广大青年学生立大志、成大才,把实现个人梦融入国家梦、民族梦之中。
叶为民所在的福建医科大学公共卫生学院,其前身是始建于1959年的福建医学院卫生系,是我国早期开设预防医学本科专业的少数院系之一,目前,该学院已发展成为以本科教育为基础、以研究生培养为重点、积极发展继续教育的多学科、多专业、多层次的福建省预防医学、食品质量与安全以及卫生检验与检疫高级人才的重要培养基地和科研中心。迄今已培养了9000多名高素质应用型公共卫生人才,许多毕业生已成为了省内外公共卫生领域科研、卫生管理单位的带头人、技术骨干与管理人才。
在叶为民的带领下,福建医科大学公共卫生学院不断创新人才培养模式和课程体系改革,构建了以创新能力培养为核心的长线式育人机制,建立起适应现代医学科学观的新型医学本科课程体系,促进公共卫生预防医学人才的创新性培养。而今,春华秋实,硕果盈枝。2018年至今,公共卫生学院获批国家级一流本科专业建设点、一级博士授予点、公共卫生与预防医学博士后科研流动站等国家级学科建设项目;获批省级虚拟仿真实验教学中心、国家级一流课程3门、省级一流课程18门、获教学成果奖14项(省级3项)、教学改革项目32项(省级8项)。近年来,本科生以第一作者发表学术论文59篇,学生获大学生创新创业课题立项111项(国家级11项、省级18项、校级72项),在创新创业学科竞赛中,荣获省级以上奖项40余项。5年来,叶为民培养了9名博士研究生和16名硕士研究生,其中第一个博士毕业生鄢灵君曾以第一作者在Gastroenterology、Phenomics、Oral oncology等领域内权威期刊发表高水平SCI论文6篇,现为福建医科大学附属第一医院第三层次A类引进人才、副研究员、脑研院PI。

“我的导师曾说过,‘科研是一场马拉松,而不是百米冲刺’。这在我准备发表博士论文时深有体会。初稿被拒后,是叶教授耐心地指导我逐条分析评审意见,并鼓励我改进研究方法,历经数月最终高质量发表在更高水平的期刊上。”鄢灵君至今记忆犹深。
“叶教授不仅是一位在学术上成就斐然的学者,更是一位全身心投入科研、注重学生培养的引路人。他一贯以身作则,工作勤奋,办公室的灯光常常是那层楼中最晚熄灭的。”叶为民的一名研究生回忆道,无数个周末、假期,当大多数人都在享受、放松时,叶教授通常都是在办公室,帮助学生、青年教师修改论文、审阅课题。“叶教授逐句推敲、反复斟酌,力求每一个数据准确可靠、每一个论点正确恰当,他对科研工作的严谨态度和专注精神让我深感敬佩,也深受影响。”
刀在石上磨,人在事上练。新冠肺炎疫情期间,叶为民动员指导百余名学生参与一线抗疫,在核酸采样、流行病学调查、病毒检测、数据统计等岗位发挥了重要作用,同时带领团队成员开展福建省公共卫生重大风险预警研究中心建设项目,为福建省疫情研判及防控措施制定等提供多项政策建议。疫情三年,叶为民教授团队边实践、边研究、边总结,在SCI源杂志上发表疫情防控论文30余篇,编写多部防疫专著和科普著作,举办近千场讲座、义诊,直接受益群众7万余人。
四十载敬业奋进,四十载上下求索。习近平总书记强调指出,“要构筑强大的公共卫生体系,完善疾病预防控制体系,建设平战结合的重大疫情防控救治体系,强化公共卫生法治保障和科技支撑,提升应急物资储备和保障能力,夯实联防联控、群防群控的基层基础。”踏上以中国式现代化全面推进中华民族伟大复兴的新征程,叶为民将继续带领团队一如既往深耕于疾病的病因流行病学与防控研究工作,将理想信念扎根祖国,把科研成果写在祖国大地上,加快提升监测预警、检验检测、应急处置等疾控核心能力,积极打造公共卫生人才培养新范式,培养一批懂技术、善管理、敢创新、识大局、有情怀且下得去、留得住、顶得上的高水平应用型公共卫生人才,用实际行动践行“人民生命健康的守护者”的承诺,助力健康中国建设迈上新的台阶。

癌症是多种因素长期综合作用的结果,其发生与环境因素及生活方式密切相关。福建省地处东南沿海消化道恶性肿瘤高发带中,学有所成的叶为民主动回到家乡谋求破局之策,以老百姓的就医需求为出发点和落脚点,依托高水平医学院校和省级高水平医院的资源及平台,推动建立“府-院-校”共建新模式,积极探索肿瘤等慢性病发病机制,开发、应用和推广防控策略和工具,在福清、莆田、泉港等地建成7.1万人的健康人群队列和1万人的临床专病队列,形成以福建省健康人群队列和数字福建肿瘤大数据研究所为依托的医疗大数据整理、分析实训平台。
“近年来,在基因组学和测序技术的飞速发展的背景下,大型自然人群队列在精准医疗领域的重要价值开始逐步凸显,日益成为国际生命科学与疾病领域的前沿与焦点。”自然人群队列研究是指通过对普通人群进行体格检查、生物样本采集和问卷调查,并进行长期持续的动态追踪观察,掌握人口健康状况、阐述疾病的发展史、揭示疾病的病因、引导实验设计、评价预防效果,将知识转化为临床和人群早期筛查诊断和干预策略,从而提高疾病的防治水平,降低社会卫生负担,为下一步针对性制定卫生方针政策提供科学依据,是疾病防控领域的重大基础战略工程。
叶为民介绍称,以大型队列为基础,整合生物样本库、遗传和基因组数据库、表型数据库等建立越来越大的生物银行正日渐成为全球的趋势。表型组与大型队列存在取与舍的问题,而精细化人群队列在有限的可接受的人群队列中,采集尽量多的表型组,以实现数量和质量的均衡。福建省自然人群队列项目规划覆盖20万人群,致力于实现精细化、高应答和低失访,“随访是队列研究成败的关键,大数据技术的发展为疾病随访率的提升提供了良好的契机,福建队列也在数据中心建设方面积累了一些经验。”
“想要做好现场,不能只在办公室待着学理论,还要去基层中实践,真正去掌握流调技能、了解数据收集过程,解决实际中遇到的困难。”这是叶为民反复强调多次的。
福清市是糖尿病、高血压、甲状腺肿瘤等疾病相对多发地区之一,亟需探究当地危险因素并采取针对性措施。2021 年11月,由福建医科大学与福清市政府组建的福建队列研究中心福清基地揭牌成立,联合开展大型前瞻性自然人群队列研究,科学探索肿瘤及多种慢性病的发生和发展规律,开展慢性病的病因预防和临床诊疗研究,为慢性病的早期筛查、诊断、个体化治疗和药物研发提供科学依据。经过一系列的探索实践,叶为民推动形成了重大慢性病防控的“福清模式”,为福建省医疗卫生事业发展奠定坚实的基础,这也意味着,更高质量、更高水平的医疗服务将极大地惠及当地百姓。2023 年 12 月,叶为民获评“福清市第二届荣誉市民”称号。
正因见证过太多“发现一例早癌、挽救一条生命、幸福一个家庭”的案例,叶为民早就将基层肿瘤防治当作了自己一生的事业。2020年12月2日,福建省队列研究泉州基地挂牌成立,由此补齐泉州地区消化道肿瘤科研、预防、综合治疗的短板,推动泉州生命科学领域研究和医疗健康核心技术取得突破,进一步带动提升泉港综合办医水平,为泉港医疗卫生事业高质量发展注入新的动能。
尤为值得一提的是,叶为民牵头推动建立以医院来源的患者医疗数据为核心的大数据平台,汇聚福州市居民健康医疗相关数据,打通队列数据与医疗数据接口,为后续诊疗大数据的应用开发和人群前瞻性队列建设奠定基础。
叶为民希望,持续推进和优化疾病预警机制,融合队列资源,关注婴幼儿、成年人、老年人全生命周期的健康监测,探索创新更加精准的疾病防控举措。
心中有民,行之为民。叶为民表示,将继续带领团队一如既往深耕于疾病的病因流行病学与防控研究工作,加快提升监测预警、检验检测、应急处置等疾控核心能力,打造公共卫生人才培养新范式,培养一批懂技术、善管理、敢创新、识大局、有情怀且下得去、留得住、顶得上的高水平应用型公共卫生人才,用实际行动践行“人民生命健康的守护者”的承诺。
